Occupational Therapist (OT) - SNF / LTC

Position Summary A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist (OT) to join their growing team on a direct‑hire permanent assignment. Responsibilities Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation. Establishing and administering a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the physician. Setting realistic and achievable goals for patients, documenting and recording the patient's condition, and educating patients and families in an appropriate occupational therapy method. Providing skilled occupational therapy services/interventions in accordance with physician orders. Assessing patient needs, planning for, evaluating and modifying care to meet the goals of occupational therapy interventions. Collaborating with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduated from an accredited school. Current state license as an Occupational Therapist (OT) or proactively in the process of applying for state licensure. Current CPR certification. #J-18808-Ljbffr
