This position involves providing services in office, home, and community settings, along with coordinating interdisciplinary care and connecting clients to appropriate evidence-based services and community resources.The Case Manager works independently to assess client needs, determine eligibility for services, develop individualized service plans, and coordinate support across medical, behavioral health, educational, and community systems. This position serves as a liaison between clients, families, and service providers to ensure timely, effective, and compassionate care delivery. In addition, the Case Manager may support the Joshua’s Path team by assisting with group activities, community outings, and other participant-centered events, contributing to program engagement and the overall participant experience.**Responsibilities:****Case Management & Care Coordination*** Manage a caseload of clients and families, providing comprehensive case management services.* Conduct psychosocial and clinical assessments to identify client and family needs.* Determine eligibility for Joshua’s Path services and external programs at the local, state, and federal levels.* Develop, implement, and monitor individualized service plans aligned with evidence-based practices.* Coordinate interdisciplinary care from intake through ongoing service delivery.* Monitor client progress and update care plans based on measurable outcomes.**Client & Family Support*** This position involves providing services in office, home, and community settings* Educate clients and families on diagnoses, services, and available interventions.* Offer ongoing guidance, emotional support, and systems navigation.* Advocate for access to services, benefits, and community resources.**Service Coordination & Referral*** Maintain a network of community providers and referral sources.* Facilitate referrals to healthcare providers, schools, early intervention, employment, residential programs, and social services.* Collaborate with multidisciplinary teams to ensure holistic client care.* Act as a liaison between families and service providers to ensure continuity of care.**Home- and Community-Based Services*** Conduct in-home visits to assess safety, well-being, and environmental factors impacting care.* Identify barriers such as housing instability, safety risks, or lack of resources.* Provide field-based support to promote independence and quality of life.* Respond to crises, including behavioral or environmental emergencies, and coordinate appropriate interventions.**Documentation & Compliance*** Maintain accurate, timely, and confidential electronic client records in accordance with agency and regulatory standards.* Complete case notes, assessments, service plans, and reporting documentation per and grant guidelines.* Track and document service utilization and outcomes.**Collaboration & Professional Engagement*** Participate in case conferences, staff meetings, and supervision sessions.* Collaborate with internal teams and external partners to enhance service delivery.* Support public education initiatives, outreach events, and community presentations.* Assist with training documentation and program reporting requirements.**Quality Assurance & Continuous Improvement*** Evaluate service effectiveness and adjust care plans as needed.* Ensure ethical conduct and client confidentiality at all times.* Exercise professional judgment in complex case decision-making.## Qualifications**Qualifications:****Education & Experience*** Bachelor’s degree in Social Work, Psychology, Special Education, Behavior Analysis, or a related field (Master’s degree preferred).* Minimum of 3–5 years of relevant experience in case management, disability services, or human services.* Experience working with individuals with disabilities and their families required.* Experience in home- and community-based services preferred.**Required Skills & Competencies*** Strong interpersonal and communication skills with diverse populations.* Demonstrated compassion, empathy, and client-centered approach.* Excellent organizational, time management, and problem-solving abilities.* Ability to work independently and manage a complex caseload.* Strong clinical judgment and decision-making skills.* Knowledge of community resources, social service systems, and care coordination practices.* Ability to collaborate effectively with multidisciplinary teams.* Proficiency in electronic documentation systems and basic technology.**Licenses & Certifications*** Valid driver’s license, reliable transportation, and current auto insurance (required).* CPR/First Aid certification preferred or willingness to obtain.**Working Conditions*** Work is conducted in a combination of office, client homes, and community settings.* Frequent local travel required for home visits and community-based service delivery.* Exposure to varied home environments, including those with safety or environmental challenges.* May require flexible scheduling to meet client needs, including occasional evenings.**Additional Duties*** Participate in training, professional development, and community outreach activities.* Maintain compliance with agency policies, grant requirements, and reporting standards.* Perform other duties as assigned by leadership.* Assist Joshua’s Path Disability Services team with various departmental needs.**Why Join Goodman JFS – Joshua’s Path**Join a mission-driven team dedicated to supporting neurodivergent adults and their families through compassionate care, advocacy, and community integration.
