YVHF Program Description
Yadkin Valley Healthy Families uses the evidence-based Healthy Families America (HFA) model to provide in-home visitation services to overburdened families who are pregnant or parenting a child, ages birth – 5 years and living in Wilkes and Yadkin Counties, NC. This service mitigates child maltreatment risks by helping families reduce stress factors and increase protective factors through supportive, regular, home visits over a span of 3-5 years. YVHF is one of many programs offered by Children’s Hope Alliance to strengthen families.
Job Summary
The YVHF Program Manager is responsible for the day-to-day hands-on management of the YVHF program serving Wilkes and Yadkin Counties, ensuring that quality services are provided to the families served. This includes staffing, training, and providing reflective supervision to all direct reports; planning and managing the budget; leading quality assurance and improvement projects; ensuring YVHF maintains positive working relationships with community partners; and maintaining ongoing collaboration with state partners.
Principal Duties and Responsibilities
Program Operations
- Maintain data entry into the Family Wise Data base and complete necessary reports for contracting purposes.
- Ensure all policies, standards, and statutes are adhered to in the program.
- Ensure the quality and quantity of work with all pertinent guidelines and standards.
- Coordinate between professionals representing various disciplines related to the client’s specific needs.
- Participate in multidisciplinary teams to coordinate the delivery of services from a variety of sources and professionals.
- Program Manager will receive reflective supervision from their direct supervisor at least monthly.
- Plan and execute quarterly Parent Groups for enrolled and graduated families which includes an educational component.
- Screen referrals to ensure they meet the YVHF enrollment criteria (family lives in service area, enrollment can occur during either the prenatal period or prior to the target child’s 3rd month of infancy).
- Provide follow-up information to referral agents and maintain contact with community referral sources.
- Communicate regularly with YVHF Advisory Committee. Schedule and lead quarterly meetings with Advisory Committee.
- Attend assigned community provider agency and service club meetings in order to collaborate and increase YVHF’s presence in the community.
- Keep YVHF staff apprised of local community events that will create greater learning opportunities for their enrolled children and community resources that may benefit the families they serve.
Documentation
- Document FROG Scale narrative and scoring and review with assigned FSS.
- Determine and document families’ consent or refusal of YVHF services.
- Document all communication with or concerning referred families in electronic database.
- Update HFA Entrance to Exit Spreadsheet with required details regarding each referral.
Administrative
- Complete requests for proposals and grant requests, as necessary.
- Monitor and review the work of the program as a whole and make necessary adjustments to meet the service delivery needs.
- Complete annual budget and monitor monthly.
- Liaison between YVHF program and community agencies to maintain cooperation/collaboration between all stakeholders.
- Participate in the planning of and attend YVHF Advisory Committee meetings.
- Develop strategies for multisystem/agency collaboration to facilitate delivery of program services and effectively utilize available resources. Make operational changes to work methods, processes and procedures in response to changes in guidelines, policies or service delivery requirements.
- Plan work operations, determine priorities, establish deadlines, and develop intervention programs.
Direct Care
- Cover open caseloads for Family Support Specialist vacation or illness as needed.
- Reassign or cover cases in the event of a transition in employment.
- Staff understand the impact of their personal belief system on delivery of services and appropriately adjust personal performance to ensure cultural sensitivity in interactions with others.

Supervision Responsibilities
- Provides reflective supervision for all direct reports including Family Support Specialist (FSS), at the frequency indicated by the Healthy Families America (HFA) Best Practice Standards.
- Document supervision notes for each direct report.
- Track date and length for each supervision session.
- Ensure each new direct report receives required training by registering them for trainings, ensuring they receive training calendars, and shadowing interactions with clients until the staff is capable of providing high quality service individually, and providing ongoing observations and written feedback to each direct report.
- Maintain training log for each direct report.
- Complete Annual Performance Evaluation for each direct report.

Education and Experience Requirements
- Master’s degree in public health or human services administration or fields related to working with children and families, or bachelor’s degree in these fields with 3 years of relevant experience, or less than a Bachelor’s degree but with commensurate HFA experience.
- A solid understanding of and experience in managing diverse staff with humility, as well as providing support to staff in stressful work environments.
- Administrative experience in human services or related field, including experience in quality assurance and continuous quality improvement.
- Willingness to engage in reflective practice (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.).
- Experience with reflective practice preferred.
- Infant Mental Health endorsement preferred.
- Knowledge of infant and child development and parent-child attachment.
- Experience with family services that embraces the concepts of family-centered and strength-based service provision.
- Knowledge of parent-infant health and dynamics of child abuse and neglect.
- Experience supporting culturally diverse communities/families.
- Experience in home visiting with a strong background in early childhood prevention services.
- Must have the ability to convey information to families, professional clinicians, and community agencies.
- Proficient computer skills to allow communication, participant record and supervision documentation, accessing of and ability to interpret information.
- Requires effective communication, decision making, interpersonal, leadership, and professionalism skills and abilities.
- Experience working with or providing services to children and families.
- An ability to establish trusting relationships.
- Acceptance of individual differences.
- Must have a valid North Carolina Driver’s License and maintain insurance coverage.
- Must submit to and pass pre-employment drug screen, criminal, HCPR and driving checks.
Physical Requirements
- Occasional driving to home visits, sometimes in outlying areas.
- Occasional exposure to the home-based environment which will require the ability to perform duties in and active environment with changes in climate and noise level.
- Reading, typing, writing, speaking, and using the telephone and prolonged sitting at a desk and working on a computer.
- The office work environment will contain slight to moderate office-related noises. The employee is not exposed to extreme weather conditions, toxic fumes, or airborne particles.
- Ability to work in home environments where household pets may be present.
