This position will ensure that the recovery coaches meet all program outcomes. This is accomplished by working closely with the clinical partner providing guidance, facilitating regular training opportunities, coordinating policies and procedures across substance use disorder treatment providers, and mentoring recovery coaches to ensure fidelity to evidence-based practices. The Program Manager will also build and maintain positive partnerships with hospital emergency departments, emergency medical service districts, substance use disorder treatment providers, recovery support centers, and homeless service providers.
AREA OF FOCUS: Funded through a four-year Rural Communities Opioid Response Program (RCORP) grant from the U.S. Department of Health and Human Services' Health Resources and Services Administration (HRSA), this full-time position supports BHN's expansion of the EPICC program into Southeast Missouri, including three counties: St. Francois, Washington and Iron. The role engages healthcare and community partners to strengthen care coordination and advance the program's goal of reducing opioid-related deaths in Missouri. The position includes BHN's full benefits package and will co-locate in the Farmington office. ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Forge and nurture partnerships with Behavioral Health Network (BHN), hospitals, Substance Use Treatment Providers, EMS Providers, and community resources to ensure person-centered care coordination.
- Manage program enrollment, ensuring persons served meet eligibility criteria and the project meets expected enrollment targets each fiscal year.
- Provide outreach and assessment for project eligibility for individuals referred to the project by hospital and EMS staff.
- Maintain frequent contact with hospital staff (ED, Behavioral Health & Medical) to receive referrals, troubleshoot challenges, coordinate care for persons served, and provide clarification and training about the EPICC program.
- Assist Outreach team members in developing recovery-oriented care plans that address behavioral health, medical, and social barriers for persons served.
- Plan and facilitate weekly Recovery Coach team meetings and ongoing training opportunities in project procedures, outreach/engagement techniques, and clinical interventions.
- Establish a local Steering Committee and facilitate monthly meetings to seek input and keep the community updated on the outcomes. A Regional Advisory Committee will also be established to solicit multi-county input, meeting twice a year.
- Engage persons served across care settings, such as transition from hospital to home. Conduct outreach efforts and provide case management, as needed. Support the development of new outreach locations in partnership with the clinical partner.
- Establish referral pathways between hospital, EMS, Community and the clinical partner.
- Establish and participate in county-wide events to provide education about substance use needs and educate on how to access the program.
- Play a consistent and active role in identifying project inefficiencies and finding collaborative solutions to problems.
- Assist BHN with program evaluation, analysis of data/system review, and production of internal/external reports.
- Maintain electronic medical records, compile reports, and complete other program documentation promptly.
- Prepare quarterly reports and other grant required reports in a timely manner.
- Master's degree in a relevant field, i.e., in social services, public health, or closely related field. (licensure is preferred)
- Demonstrated knowledge and skills in project planning and coordination, and policy development.
- 5+ years of supervisory experience and project coordination.
- 5+ years of experience in intervention programs serving clients with behavioral health needs and socio-economic vulnerabilities.
- Knows effective collaboration, coordination, and facilitation strategies with demonstrated experience in applying this knowledge to real-world situations in community networks with diverse stakeholders.
- Analytical skills and prior experience using data reports to drive process and decision-making. Highly comfortable working with outcomes and metrics.
- Superior leadership, organizational, and communication skills. Evidence of initiative and attention to detail. Comfortable making public presentations and managing meetings. Strong verbal, written, and interpersonal skills.
- The applicant has strong computer skills, particularly in Microsoft PowerPoint, Word, and Excel.
- Excellent customer service sensibility.
- Working knowledge of the public behavioral health safety net, service delivery systems, and stakeholders related to behavioral health services in Washington, Iron and St. Francois counties.
- Experience working with Community Mental Health Centers (CMHCs), Substance Use Treatment Centers and the Missouri Department of Mental Health (DMH).
- Experience working on an externally-funded grant initiative, ideally implementing services/intervention grants with an evaluation, to include federal grants (like SAMHSA or HRSA).
- Experience working on a multi-agency collaborative initiative.
- Strong group training and facilitation skills, preferably including the provision of staff support to an oversight Committee.
- Knowledge of substance use resources and the field of recovery.
- Hybrid work schedule, with much work being done 'in the field' / out in the community.
- Environmentally controlled office environment, located in Farmington, MO (St. Francois County).
- Must have and maintain a valid driver's license (with no more than 2 points on the record) and current insurance
- Required ability to drive to and from meetings in the 3-county region of Washington, Iron, and St. Francios County, with periodic meetings and trainings in St. Louis.
