Position Overview

Seeking a compassionate and dedicated Registered Nurse (RN) Hospice Case Manager to provide exceptional hospice care throughout the Greater Lake Charles area. The RN Case Manager will oversee a caseload of patients, coordinating and delivering personalized care plans that support patients and families during their hospice journey. This position is ideal for a nurse who is patient-centered, adaptable, and committed to providing high-quality end-of-life care.

Schedule & Work Details

• Coverage Area: Greater Lake Charles
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hour Guarantee: 40 hours
• Pay Rate: $51.00 - $53.00 per hour
• On-Call Requirement: None
• Weekend Requirement: None
• Mileage Reimbursement: Yes, mileage is reimbursed between patient visits

Key Responsibilities

• Serve as the primary case manager for an assigned group of hospice patients.
• Complete comprehensive patient assessments and develop individualized plans of care.
• Coordinate care with physicians, interdisciplinary team members, patients, and family caregivers.
• Monitor patient conditions and make appropriate care plan adjustments based on changing clinical needs.
• Provide skilled nursing interventions and symptom management to promote patient comfort and quality of life.
• Educate patients and families regarding disease processes, medications, hospice services, and end-of-life care.
• Maintain accurate, timely, and compliant clinical documentation.
• Advocate for patients and families while supporting their physical, emotional, and psychosocial needs.
• Ensure all care is delivered in accordance with hospice regulations, policies, and standards of practice.

Required Qualifications

• Active and unrestricted Registered Nurse (RN) license.
• Previous hospice nursing experience.
• Strong clinical assessment and case management skills.
• Excellent communication, organizational, and interpersonal abilities.
• Ability to work independently while managing a patient caseload effectively.

• Experience with Homecare Homebase (HCHB).
• Patient-centered approach to care delivery.
• Positive attitude with a willingness to learn and adapt.
• Flexible, dependable, and team-oriented mindset.
