CNA - Home Hospice, Full TimeLocation: Johnson Creek, WI 53094DescriptionLocation: Driving in the Field Position Type: Full Time, 1.0 FTE, 40 Hours per Week Schedule Requirements: Primarily day shift, but also includes a required on-call, weekend, and holiday rotation. Bonus: This position is BONUS Eligible at 6 months of successful employment. Bonus range is $3,000 to $5,000 depending on hospice experience.Be the Difference! Use your skills to provide high quality home care to terminally ill patients. The position involves working in different working environments, including patients' homes and healthcare facilities.Key ResponsibilitiesAssists with ambulation and transfers of patient.Maintains a clean, safe and comfortable environment.Prepares meals and assists patients with eating.Gives emotional and psychological support to the patient/family.Reports status/observations of assigned patients according to RCC guidelines.Promotes patients' independence.Maintains up-to-date patient records and documents care given and patient condition for each visit.Assists the patient in activities of daily living including personal grooming, feeding, drinking, toileting, bathing, skin care, nail care, dressing, transferring and walking.
