Case Manager - Youth

Starting Pay: $23.00 per hour Paid Time Off (PTO) Starting at 16 Days/Year Medical with federal minimum deductibles Dental and vision coverage Retirement planning and employer contribution Position Summary: The Case Manager provides direct support to youth and families through health interventions to assigned clients with severe and persistent mental illness who require individual community based rehabilitative services and/or group rehabilitative services to maximize the reduction of symptoms of mental illness in order to restore clients to the highest level of functioning. Key Responsibilities & Role Highlights: Provides Therapeutic Behavioral Services to an assigned caseload of clients in accordance with the State mandates and established Agency and program policies, procedures, philosophy and client individual service plan. Provides transportation to and from various locations in support of the assigned program. Documents all services provided and maintains records utilizing the electronic record in accordance with agency policy/procedure Participates in team meetings, supervision, in-service training, and conferences according to established policies, procedures, and applicable regulations. Maintains client confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures. Education & Experience Requirements: Requires a bachelor's degree in related field. Experience with youth preferred. Must demonstrate knowledge of mental illness including the SED population, crisis management and the ability to establish and maintain therapeutic relationships with clients. Must demonstrate the ability to define problems, analyze data, establish facts, draw valid conclusions, and make recommendations. Must demonstrate positive attitude, commitment to provide superior services and enthusiastic support for the mission of the agency. Must establish and maintain effective working relationships. Must demonstrate the ability to represent the agency in a positive manner when in the community. Must be able to work with ill, disabled, emotionally upset and at times hostile individuals.
