Care Coordinator

The Care Coordinator is responsible for frontline communication with patients by phone or in person and supports CHC clinical operations with ongoing care of pediatric and adult patients. The Care Coordinator supports the Clinic Supervisor. Location Shipshewana, IN – The Community Health Clinic, Inc. (CHC) is a 501(c)(3) non‑profit healthcare facility that will provide medical care consistent with the needs of the Amish, Mennonite, and rural northern Indiana communities. The CHC will serve children and adults with special healthcare needs. Essential Functions and Responsibilities To perform this job successfully, an individual must be able to perform each duty satisfactorily. Administrative Functions Obtain preauthorization for visits for Children with Special Healthcare Services for Indiana and Obtain preauthorization for diagnostic testing and medications. Process clinical information and Lab/Diagnostic requests. (Scheduling tests and providing necessary paperwork) Attend staff meetings Attend patient education events as needed. Maintain patient information and update spreadsheets as directed Understand what a medical home is, and the services it provides, and work with families to establish a medical home. Coordinate care with other PCPs, HCPs when a family chooses an outside medical home Prepare appropriate documents/materials for the patient to complete (pre‑visit letters/calls) Educate families on the benefits of a medical home (pre‑visits, time of service, telephone calls) Organize care for patients choosing to use the CHC as their medical home If referrals are made by the CHC to another provider, help schedule appointments and transfer records to referred source. Address potential barriers that would prevent family from following through. Help patients with financial issues by developing an open relationship with the Plain Church Group Ministry when applicable. Refer families to other agencies that can help provide fluid, coordinated services, included, but not limited to First Steps, ARC, insurance. Research pricing for diagnostics by insurance provider/self‑pay for patients Conducts clinical follow‑up calls and coordinates referral appointments. Clinical Support Functions Provides care to patients by documenting vital signs, chief reason for visit, new problems, medical/family history, and other nursing information. Organizes medical home support at patient homes and outreach clinics in an effective and Draw ordered laboratory studies Respond to patient telephone calls Arrange pharmacy refills Other Functions Performs other related duties and responsibilities as directed Complies with HIPAA regulations Complies with OSHA regulations Develops and maintains cultural knowledge base and skills to sensitively interact with local and regional populations Education and/or Experience To perform this job successfully, an individual should have the following education and/or experience. Certified Medical Assistant Alternative to minimum qualifications: Medical Assisting Diploma from an accredited institution plus 2 years work experience in a physician’s office performing clinical duties OR working in the role of Medical Assistant in a physician’s office performing front and back‑office duties for a minimum of 6 years. Proficient in medical terminology Organizational skills Knowledge, Skills, and Abilities The requirements listed below are representative of the knowledge, skills and/or abilities required to perform each essential duty satisfactorily. Ability to work in a fast‑paced clinical environment Ability to push, pull, lift, move, and/or carry up to 25 lbs.
