Occupational Therapist

Conduct standardized occupational therapy assessments and evaluations in clients' homes and community settings.

Develop and implement individualized treatment plans based on clients' needs, goals, and functional abilities.

Provide weekly therapeutic interventions to improve daily living skills, independence, and community participation.

Maintain accurate and timely documentation, including evaluations, daily progress notes, and discharge summaries with recommendations.

Upload all evaluations and progress notes to the designated system daily.

Participate in interdisciplinary team meetings and provide updates on treatment goals, interventions, and client progress.

Collaborate with the clinical team and clients to develop and coordinate therapy schedules.

Submit weekly written summaries of completed tasks and activities.

Maintain accurate weekly work logs, activity logs, and monthly therapy attendance records.

Complete weekly mileage and expense reports related to travel and therapy services.

Attend weekly management meetings and collaborate with team members to support quality patient care.

Maintain professional, organized, and confidential client records.

Master's degree in Occupational Therapy from an accredited university.

Current and valid Michigan Occupational Therapist license.

Knowledge of occupational therapy principles and experience working with individuals with traumatic brain injuries (TBI).

Experience or familiarity with neurological conditions and mental health-related treatment needs preferred.

Strong assessment, treatment planning, problem-solving, and clinical documentation skills.

Excellent verbal and written communication skills.

Ability to work independently and collaboratively as part of an interdisciplinary team.

Proficiency with computers, electronic documentation, and related software.

Strong organizational skills, attention to detail, and ability to manage multiple priorities.

Reliable transportation, a valid Michigan driver's license, and current automobile insurance.

Prior occupational therapy experience demonstrating competency in clinical responsibilities preferred.

This is a full-time, in-person position providing services to adult clients in home and community-based settings. The Occupational Therapist must be comfortable traveling between Troy and Utica, Michigan, and working with clients who have neurological and mental health-related needs.
