The Care Coordinator plays a vital role in ensuring consumers receive seamless, integrated care by connecting them with behavioral health, physical health, and community-based resources that promote wellness, recovery, and overall quality of life.This position works collaboratively with consumers, providers, and community partners to coordinate services and support a person-centered plan of care.Education RequirementsMinimum Associate Degree or Diploma in Nursing requiredCurrent Kentucky nursing license requiredExperience RequirementsExperience working with and accessing community resourcesProficiency in accurate and timely documentation within an electronic health record (EHR)Demonstrated ability to collaborate effectively with multidisciplinary teamsExperience working with special populations served under CCBHC guidelines preferredStrong organizational skills and ability to manage multiple priorities simultaneouslyExperience in public health, care coordination, social services, or behavioral health settings preferredEssential Duties and ResponsibilitiesCoordinate services for consumers across physical health, behavioral health, and community support systemsFacilitate linkages to community resources, including: Social servicesHousing assistanceEducational programsEmployment servicesHealthcare providersOther support services as neededEnsure smooth transitions of care between providers and levels of serviceDevelop and maintain collaborative relationships with community partners and service providersSupport consumers in achieving goals identified in their person-centered plan of careDocument all care coordination activities accurately and timely within the electronic health recordMonitor consumer progress and assist in addressing barriers to treatment and recoveryParticipate in multidisciplinary treatment team meetings and case consultationsMaintain compliance with CCBHC standards, agency policies, and regulatory requirementsAdvocate for consumer needs while promoting independence, wellness, and recoveryKnowledge, Skills, and AbilitiesStrong interpersonal and communication skillsAbility to build rapport with consumers from diverse backgroundsKnowledge of community resources and referral processesExcellent organizational and time management skillsAbility to maintain confidentiality and professional boundariesProficiency with electronic health records and Microsoft Office applicationsCommitment to person-centered, trauma-informed, and recovery-oriented care
