Requirements:

• Board Certified or Board Eligible in Urology
• Active Arizona medical license or IMLC Letter of Qualification required at time of submission
• Active DEA certification required
• Minimum of 1 year of clinical experience
• COVID-19 and Flu vaccinations required (medical/religious exemptions considered on a case-by-case basis)
• Current case logs and vaccination records required for credentialing
• Completion of Cerner EMR training (approximately 4 hours, remote)
