In this full-time, onsite role, you will function within a collaborative outpatient practice that emphasizes patient-centered care under the leadership of the Chief Nurse Practitioner and in partnership with our Medical Director. Key Responsibilities

As a Nurse Practitioner in our Cardiology Clinic, your core responsibilities will include:

• Conducting thorough patient assessments by obtaining detailed medical histories and performing comprehensive, targeted physical examinations.
• Ordering, interpreting, and acting on diagnostic tests to correctly diagnose and manage both chronic and acute cardiac issues.
• Formulating and implementing goal-directed care plans that align with established protocols, focusing on health maintenance and anticipatory guidance.
• Working alongside physicians to collaboratively adjust treatment plans based on patient progress and follow-up discussions.
• Counseling and educating patients and their families about their conditions, treatment options, and effective care management strategies.
• Coordinating with inpatient clinical teams to facilitate seamless hospital admissions and secure safe discharges.
• Documenting all patient interactions and care delivered in our electronic medical record (EMR) system, ensuring accuracy and compliance with regulatory standards.
• Participating in research initiatives, integrating findings into clinical practice to enhance treatment protocols and patient outcomes.
• Maintaining a clinical environment that prioritizes patient safety, comfort, and satisfaction.
• Carrying out additional responsibilities as assigned, contributing to overall clinic efficiency.

Patient Population / Client Focus

This position primarily focuses on adult cardiac patients, including those dealing with advanced heart failure, pulmonary arterial hypertension, and post-heart transplantation. Your work will involve critically ill patients and those requiring advanced therapies such as mechanical circulatory support, intravenous, or inhaled medication therapy. Understanding the uniqueness of each patient’s journey will enable you to tailor your approach accordingly.

Qualifications

To be considered for this role, candidates must possess the following qualifications:

• A Master’s degree in Nursing with specialization as a Nurse Practitioner, specifically trained in adult care.
• A minimum of two years of relevant clinical experience in a hospital or outpatient setting, preferably in Cardiology.
• Current New York State licensure as a Registered Professional Nurse and Nurse Practitioner.
• Board certification as a Nurse Practitioner.
• Up-to-date Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), DEA, and National Provider Identifier (NPI) certifications.
• Proficiency in electronic medical record systems and common software applications such as MS Word, Excel, and PowerPoint.

Preferred Experience

While not mandatory, candidates with the following experiences will be favored:

• Experience working with patients undergoing advanced therapies, including intravenous or inhaled medication support.
• A background in handling complex cardiac cases and knowledge of advanced heart conditions.

Compensation and Benefits
