Family Services Coordinator Job Description Template
Our company is looking for a Family Services Coordinator to join our team.
Responsibilities:
- Support Families of Children with Chronic Health Conditions and Special Needs;
- Outcomes-Based Family Services;
- Promote Family Life Practices to Close the Achievement Gap;
- Links clients and their families to appropriate resources and services in a timely and coordinated manner;
- Provides advocacy for the development of services and resources that do not currently exist within the community but are deemed necessary;
- Participates in assigned meetings, events and training as required;
- Competency-Based Assessment, Coaching and Supervision;
- Conducts outreach to engage clients and assist families with an understanding of agency philosophy and policies;
- ERSEA (Eligibility, recruitment, selection, enrollment, and attendance);
- Attends agency in-services, workshops and seminars as required;
- Responsible to travel within the county and meet with clients within homes and schools;
- Identifies and utilizes both formal and informal community supports and networks;
- Attends family court as necessary;
- Schedules monthly team meetings with the family, service providers and any other individuals identified as a team member;
- Acts as a liaison between client and family, agency personnel, DSS staff and other service providers as necessary and appropriate.
Requirements:
- Adhere to all confidentiality requirements;
- Fulfillment of TB test, background check clearance, and any other mandatory State/Federal requirements;
- Ability to work cooperatively in a team environment within department and agency wide;
- Verbal and written ability to communicate effectively with colleagues, community partners, consumers and all levels of management;
- Proven documentation skills and intermediate level computer skills including all MS Office programs;
- Prepared to work independently with flexible hours that may include evenings, weekend and holidays;
- Ability to multi-task and prioritize projects and assignments effectively;
- Valid California driver’s license, proof of automobile insurance, can maintain a driving record acceptable to the agency’s insurance carrier;
- Must provide and operate a registered, properly insured automobile.
