Our company is looking for a Social Work Care Coordinator to join our team.

Responsibilities:

  • Case closure as appropriate;
  • Complete all required documentation in a timely manner according to department policy;
  • Attend NAMM Case Management meeting and trainings as appropriate;
  • Telephonically monitor patient on a regular basis in regards to psychosocial needs, education, and compliance with treatment plan;
  • Refer patient to outside community resources, classes, education, etc. as appropriate;
  • Acts as a resource person for other members of the healthcare team;
  • Telephonically assess patient’s psychosocial status;
  • Develop and maintain social service program, drafting supporting policies, procedures, and guidelines for the program;
  • Maintains up to date knowledge of Medicare, Medical, and other third party reimbursement sources;
  • Collect data necessary for NAMM Case Management reports / QI studies;
  • Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems;
  • Refer to health education and other service departments as necessary;
  • Develops and maintains through knowledge of referral resources throughout the service area;
  • Provide social work services including, short – term counseling, community resource planning, and crisis intervention;
  • Report to NAMM Case Management Nurse variances in patient’s condition, medications, or treatment.

Requirements:

  • Graduate of an accredited Master’s degree social work program;
  • Ability to work in a nurse managed, multi – disciplinary team;
  • Excellent interpersonal and communication skills, both written and oral;
  • At least one year of social work experience in a healthcare setting;
  • MSW or LCSW;
  • 2+ years of clinical experience with broad range of experience in health care delivery setting (hospital, clinic, physician’s office);
  • Strong critical thinking and decision-making skills;
  • Computer knowledge and proficiency;
  • Access to reliable means of transportation to travel in designated area.
