Case Manager – Per Diem Job Description Template
Our company is looking for a Case Manager – Per Diem to join our team.
Responsibilities:
- Develop and implement a patient specific, safe and timely discharge plan;
- Facilitate multi-disciplinary team meetings including physicians, nurses, respiratory therapists and rehabilitation therapists;
- Perform verification of utilization criteria reviews.
Requirements:
- Facilitate multi-disciplinary team meetings including physicians, nurses, respiratory therapists and rehabilitation therapists;
- Perform verification of utilization criteria reviews;
- Develop and implement a patient specific, safe and timely discharge plan;
- Passion for providing superior quality of care and excellent organizational and communication skills;
- Experience in care management, discharge planning, and utilization review;
- Working knowledge of the insurance industry and government reimbursement;
- Registered Nurse (RN) or Licensed Social Worker (SW);
- Availability to work on an “as needed” basis – shifts would be scheduled on weekdays, Monday-Friday from 8am-5pm;
- Registered Nurse (RN) preferred;
- Ability to provide coverage both Milwaukee locations (St. Francis and West Allis) on an as needed basis;
- Registered Nurse (RN) or Licensed SW;
- Flexibility to provide coverage on an as needed basis, shifts would be Monday-Friday between 8am-5pm.
