Our company is looking for a Clinical Case Manager to join our team.

Responsibilities:

  • Collaborate with outside treatment teams and families;
  • Assist with educational and/or vocational goals;
  • Provide case management services as part of a multidisciplinary team;
  • Participate in completing intake assessments, monthly treatment meetings, and aftercare planning;
  • Familiarity and liaison with community resources;
  • Act as a primary case manager to up to 4 residents;
  • Participate in daily meetings/community groups as defined by the program;
  • Medication management and administration;
  • Document daily progress notes for all residents on case load;
  • Develop treatment plans with residents related to substance use/ mental health needs.

Requirements:

  • Perform other job related responsibilities, tasks and projects as needed;
  • Maintain appropriate documentation and files to support the services that are provided to families;
  • Participate in staff and supervision meetings, and required Woodbourne Center in-service meetings;
  • Ensure confientiality of all treatment and services information pertaining to clients and their foster, adoptive and biological families;
  • Understands and supports the Woodbourne Center and Nexus Philosophy and mission;
  • Supervise the placement and ongoing support of youth placed in foster and adoptive homes assigned to their caseload.
