Clinical Case Manager Job Description Template
Our company is looking for a Clinical Case Manager to join our team.
Responsibilities:
- Collaborate with outside treatment teams and families;
- Assist with educational and/or vocational goals;
- Provide case management services as part of a multidisciplinary team;
- Participate in completing intake assessments, monthly treatment meetings, and aftercare planning;
- Familiarity and liaison with community resources;
- Act as a primary case manager to up to 4 residents;
- Participate in daily meetings/community groups as defined by the program;
- Medication management and administration;
- Document daily progress notes for all residents on case load;
- Develop treatment plans with residents related to substance use/ mental health needs.
Requirements:
- Perform other job related responsibilities, tasks and projects as needed;
- Maintain appropriate documentation and files to support the services that are provided to families;
- Participate in staff and supervision meetings, and required Woodbourne Center in-service meetings;
- Ensure confientiality of all treatment and services information pertaining to clients and their foster, adoptive and biological families;
- Understands and supports the Woodbourne Center and Nexus Philosophy and mission;
- Supervise the placement and ongoing support of youth placed in foster and adoptive homes assigned to their caseload.
