Community Health Worker Job Description Template
Our company is looking for a Community Health Worker to join our team.
Responsibilities:
- Conduct post discharge activities from hospital facility and support connection to social services;
- Support the member to ensure pick-up of their prescriptions;
- Provide member education on community resources and benefits;
- Proactively engage the member to manage their own health and healthcare;
- Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician;
- May conduct Health Risk Assessments (HRA) if needed;
- Create a positive experience and relationship with the member;
- Help member set goals and develop a care plan to achieve those goals;
- As needed, help the member engage with mental health and substance use treatment;
- Partner with care team (community, providers, internal staff);
- Knowledge and continued learning of community cultures and values.
Requirements:
- Knowledge of culture and values of community;
- CHW Accreditation;
- Work experience in clinical sales;
- Ability to access high-speed internet (Provided by UHG);
- Must currently reside in one of the following counties: Norfolk, Virginia Beach, Portsmouth, Chesapeake, Hampton;
- Ability to travel locally, up to 100 miles round trip (50 miles one way) and up to 80% of the time (includes mileage reimbursement);
- Knowledge of Medicaid and/or Medicare population;
- Ability to comply with UHG Telecommuting Policy;
- Familiarity with the resources available in the community;
- Reside in Virginia for 2+ years;
- Strong desire to grow a career in health care;
- Excellent problem solving/analytical abilities to independently resolve a variety of routine and complex issues;
- Demonstrated ability to work collaboratively in a team structure;
- Bachelor’s degree;
- Excellent written and verbal communication skills.
