Our company is looking for a Community Health Worker to join our team.


  • Conduct post discharge activities from hospital facility and support connection to social services;
  • Support the member to ensure pick-up of their prescriptions;
  • Provide member education on community resources and benefits;
  • Proactively engage the member to manage their own health and healthcare;
  • Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician;
  • May conduct Health Risk Assessments (HRA) if needed;
  • Create a positive experience and relationship with the member;
  • Help member set goals and develop a care plan to achieve those goals;
  • As needed, help the member engage with mental health and substance use treatment;
  • Partner with care team (community, providers, internal staff);
  • Knowledge and continued learning of community cultures and values.


  • Knowledge of culture and values of community;
  • CHW Accreditation;
  • Work experience in clinical sales;
  • Ability to access high-speed internet (Provided by UHG);
  • Must currently reside in one of the following counties: Norfolk, Virginia Beach, Portsmouth, Chesapeake, Hampton;
  • Ability to travel locally, up to 100 miles round trip (50 miles one way) and up to 80% of the time (includes mileage reimbursement);
  • Knowledge of Medicaid and/or Medicare population;
  • Ability to comply with UHG Telecommuting Policy;
  • Familiarity with the resources available in the community;
  • Reside in Virginia for 2+ years;
  • Strong desire to grow a career in health care;
  • Excellent problem solving/analytical abilities to independently resolve a variety of routine and complex issues;
  • Demonstrated ability to work collaboratively in a team structure;
  • Bachelor’s degree;
  • Excellent written and verbal communication skills.
