Home Health RN Case Manager Job Description Template
Our company is looking for a Home Health RN Case Manager to join our team.
Responsibilities:
- OASIS C Documentation for all Home Health Visits including Start of Cares/Admissions;
- Case management in patient’s homes in the Kennesaw, GA area;
- Case management in patient’s homes in the Marietta, GA area;
- Case management in patient’s homes in the Pleasant Hill, CA area;
- Case management in patient’s homes in the King, NC area;
- Case management in patient’s homes in the Butler, PA area;
- Case management in patient’s homes in the College Park, GA area;
- Ensures that arrangements for equipment and other necessary items and services are available;
- Manage and supervise LPNs performance in implementing nursing services;
- Instructs, supervises, and evaluates home health aide care provided at least every two weeks;
- Counseling the client and family in meeting nursing and related needs;
- May teach and train other nursing personnel. Attends in-services and department meetings as required;
- Regularly re-evaluates the client’s nursing needs and revises the plan of care as necessary;
- Complete review and reconciliation of medications, pain management, and evaluation of risk status;
- Support and uphold Haven Home Health policies and procedures.
Requirements:
- Associates or Bachelor’s degree from an accredited college/university;
- Basic Life Support certification;
- Current and valid Georgia Registered Nurse License;
- Current and valid California Registered Nurse License;
- Current and valid North Carolina Registered Nurse License;
- Current and valid Georgia Certification;
- Current and valid Maryland Registered Nurse License.
