Hospice RN Case Manager Job Description Template
Our company is looking for a Hospice RN Case Manager to join our team.
Responsibilities:
- Document patient status and communicate appropriate with Interdisciplinary team members and/or physicians;
- Assess patient and family needs to provide comprehensive plans of car;
- Coordinating end-of-life healthcare and social services for patients and their families;
- Assisting patients and families in appropriate continuity of care plans, in-home and in the facility;
- Support and uphold Haven Hospice policies and procedures;
- Furnishes services requiring substantial and specialized nursing skill. Initiates appropriate palliative nursing procedures;
- Participates in IDG team meetings/care conferences and communicates with other hospice team members involved in care;
- Manage and supervise LPNs and CNAs performance in implementing nursing services;
- Regularly re-evaluates the client’s nursing needs and revises the plan of care as necessary;
- Performing on-call responsibilities as directed by Agency Management;
- Providing nursing care to patients as prescribed by the physician and/or approved care plan;
- Instructs, supervises, and evaluates home health aide care provided at least every two weeks;
- Supervising ancillary personnel and delegates, as needed;
- Aiding patients in daily living tasks to maximize comfort and enable increased self-sufficiency;
- Implements teaching specific to patient/family needs and evaluates and modifies as needed.
Requirements:
- Must have at least 2 years experience as a Registered Nurse;
- All Nurses must have reliable transportation with current automobile insurance;
- Candidates must be able to pass a criminal background check and drug screening;

- Current Texas Driver’s License and auto insurance;
- Flexible and cooperative in fulfilling all obligations;
- CHPN preferred but not required;
- Auto liability insurance per policy plus current and unrestricted driver’s license;
- At least one year of nursing experience, hospice experience is a plus;
- Ability to work in an interdisciplinary setting;
- Compliance with accepted professional standards and practices;
- Presentation of negative TB test or CXR as required by state law;
- Graduate of an accredited RN program;
- Demonstrate excellent observation, good nursing judgment and communication skills.
