Our company is looking for a Registered Nurse Case Manager to join our team.
Responsibilities:
- Consult with doctors and other healthcare professionals;
- Set up patient medicine and provide treatments;
- Participate patient/family care conferences;
- Manage after hours, emergency patient calls and visits;
- Provide education/instructions to patients and families regarding disease and/or symptom management;
- Admit patients to hospice services;
- Documents care provided, observations made and communications with patient, family and facility caregivers in electronic medical record.
Requirements:
- Associates/Bachelor’s degree;
- Current RN license in State of MN;
- Current CPR Certification for health care providers.
