Our company is looking for a RN Care Manager to join our team.

Responsibilities:

  • Documents clinical activity in the medical record and supports and coaches accurate clinical documentation by physicians and others;
  • Builds a network of positive working relationships that advocate for the patient;
  • Holds, interprets, and integrates the patient?s story into the overall multidisciplinary plan of care;
  • Performs other duties as assigned;
  • Reviews all patients identified for review to determine the medical necessity and appropriateness of enrollment in the care management program;
  • Actively participates in clinical performance improvement activities as assigned;
  • Pursues continuing education in area of expertise/leadership;
  • Palliative/Crisis care, supervision of CNAs, STNAs;
  • Other duties as requested by supervisor;
  • Medical;
  • Case Management, Assessments, Assisting Care Partners, Coming Home and On-Call Admissions.

Requirements:

  • Excellent interpersonal and communication skills, both verbal and written;
  • Must hold a current, valid Arizona or compact state RN license;
  • Experience with INTERQUAL or Milliman;
  • Electronic Medical Record (EMR) experience preferred; Cerner strongly preferred;
  • Computer literacy and proficiency in Microsoft Windows;
  • Strong decision making and problem solving skills;
  • Knowledge of regulatory requirements necessary to integrate into patient care delivery system;
  • Basic proficiency with MS Office (Outlook, Word, Excel);
  • Knowledge and skills necessary to evaluate appropriate care for patients from neonates to geriatrics;
  • CCM certification;
  • Knowledge of Title XXII and JCAHO requirements;
  • Excellent interpersonal skills and the ability to effectively communicate verbally and in writing providing excellent customer service;
  • Demonstrates and models professionalism, critical thinking/judgment and partnership in all interactions;
  • Demonstrated time management skills;
  • Bachelor’s degree in Nursing.
