RN Care Manager Job Description Template
Our company is looking for a RN Care Manager to join our team.
Responsibilities:
- Documents clinical activity in the medical record and supports and coaches accurate clinical documentation by physicians and others;
- Builds a network of positive working relationships that advocate for the patient;
- Holds, interprets, and integrates the patient?s story into the overall multidisciplinary plan of care;
- Performs other duties as assigned;
- Reviews all patients identified for review to determine the medical necessity and appropriateness of enrollment in the care management program;
- Actively participates in clinical performance improvement activities as assigned;
- Pursues continuing education in area of expertise/leadership;
- Palliative/Crisis care, supervision of CNAs, STNAs;
- Other duties as requested by supervisor;
- Medical;
- Case Management, Assessments, Assisting Care Partners, Coming Home and On-Call Admissions.
Requirements:
- Excellent interpersonal and communication skills, both verbal and written;
- Must hold a current, valid Arizona or compact state RN license;
- Experience with INTERQUAL or Milliman;
- Electronic Medical Record (EMR) experience preferred; Cerner strongly preferred;
- Computer literacy and proficiency in Microsoft Windows;
- Strong decision making and problem solving skills;
- Knowledge of regulatory requirements necessary to integrate into patient care delivery system;
- Basic proficiency with MS Office (Outlook, Word, Excel);
- Knowledge and skills necessary to evaluate appropriate care for patients from neonates to geriatrics;
- CCM certification;
- Knowledge of Title XXII and JCAHO requirements;
- Excellent interpersonal skills and the ability to effectively communicate verbally and in writing providing excellent customer service;
- Demonstrates and models professionalism, critical thinking/judgment and partnership in all interactions;
- Demonstrated time management skills;
- Bachelor’s degree in Nursing.
