Our company is looking for a RN Case Manager to join our team.

Responsibilities:

  • Counseling the client and family in meeting nursing and related needs;
  • Providing those services and/or treatments requiring substantial and specialized nursing skill;
  • Planning for the discharge from service;

  • Provides age and culturally appropriate care;
  • Performs utilization review ensuring admissions meetcriteria for appropriateness of care and medical necessity;
  • Talking with the patient and their family members to diagnose their needs;
  • Utilizes the principles of case management in delivering effective services for a caseload of hospice patients and families;
  • Route calls to appropriate department for continued assistance as necessary;
  • Take complete on-line messages that are detailed, concise, spelled accurately and utilize correct medical terminology;
  • Participates in the agency’s QI – Performance Improvement Program;
  • Follows HIPAA regulations regarding patient information;
  • Effectively and efficiently manages resources (supplies, medications, and equipment);
  • Maintain thorough knowledge and understanding of PPPSW organization in order to answer questions and direct calls correctly and appropriately.

Requirements:

  • Active RN License from California;
  • Case Management Experience;
  • One or more years Registered Nurse experience with at least one year hospital medical-surgical/ oncology experience preferred or;
  • Possession of a two year Associate Degree in nursing or a Bachelor’s Degree in nursing from an accredited school of nursing program,
  • Actively and positively strive to implement the goals of the Case Management Program and support changes;
  • Bilingual (English/Spanish) preferred;
  • 1 year of experience in Women’s Healthcare;
  • Registered Nurse with a minimum of one (1) year of home health experience;
  • Has the ability to work independently and within an interdisciplinary team;
  • Must possess and maintain valid CPR certification while employed in a clinical role;
  • Must be currently licensed as a registered nurse in the state of agency operation. Bachelors degree preferred;
  • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements;
  • Current California RN license;
  • Demonstrates strong organizational, time management, and prioritization skills;
  • Establish an effective working relationship with internal and external customers.
