Our company is looking for a Social Services Assistant to join our team.
Responsibilities:
- Comply with the Residents’ Rights and Facility Policies and Procedures;
- Good assessment, listening, and counseling skills;
- Perform Essential Duties as outlined above;
- Minimum Qualifications;
- Work with the facility’s consultants as necessary and implement recommended changes as required;
- Attend in-service education programs in order to meet facility educational requirements;
- Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident;
- Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc;
- Involve the resident/family in planning social service programs when possible;
- Good communication skills with excellent self-discipline and patience;
- Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident;
- Participate in the community planning related to the interest of the facility and the services and needs of the resident and family;
- Compile information on discharge plans and present to appropriate committee as required;
- Participate in discharge planning, development and implementation of social care plans and resident assessments;
- Make written and oral reports/recommendations to the Director concerning the operation of the social services department.
Requirements:
- Maintains confidentiality of all proprietary and/or confidential information;
- Must understand and follow company policies including harassment and compliance procedures;
- Must possess the ability to make independent decisions when circumstances warrant such action;
- Must perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation;
- Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department;
- Maintains professional working relationships with all associates, vendors, etc;
- Must be a high school graduate or equivalent;
- Prior health care experience preferred;
- Organized and detailed in work performance;
- Good communication skills with excellent self-discipline and patience;
- Genuine caring for and interest in elderly and disabled people in a nursing facility;
- Comply with the Residents’ Rights and Facility Policies and Procedures;
- Good assessment, listening, and counseling skills;
- Perform work tasks within the physical demand requirements as outlined below;
