Our company is looking for a Peer Mentor to join our team.

Responsibilities:

  • Other duties as assigned;
  • Support for the child and his/her family;
  • Be a member of the family team to assist in the strengths assessment and wrap plan development;
  • Increase collaboration with families and professionals.

Requirements:

  • Ability to travel to classrooms to promo program, gather student feedback or host focus groups;
  • Continue to learn and share information, resources and certification processes;
  • Ability to adjust work priorities per program needs;
  • Availability to work weekends and evenings as needed;
  • Host various in-person and web conferences to facilitate group discussion and skills demonstration;
  • Perform needs assessments with mentees and co-develop action plans to ensure the services provided are on track;
  • Attend all team meetings, trainings and events;
  • Schedule mentor sessions, follow up sessions, track and document the provision of services in a timely manner;
  • Use multiple computer systems to track and review documentation in our database and perform daily tasks;
  • Properly document and track data in various electronic databases;
  • Discuss mentees’ concerns and trends with the Leads and Managers;
  • Serve as an energetic and enthusiastic mentor for the Home Care Aides;
  • Additional duties as assigned by your manager;
  • Proactively make outreach phone calls to offer support and guidance to other Caregivers on various needs;
  • Recruit, coach and support other caregivers over the phone.
