Peer Mentor Job Description Template
Our company is looking for a Peer Mentor to join our team.
Responsibilities:
- Other duties as assigned;
- Support for the child and his/her family;
- Be a member of the family team to assist in the strengths assessment and wrap plan development;
- Increase collaboration with families and professionals.
Requirements:
- Ability to travel to classrooms to promo program, gather student feedback or host focus groups;
- Continue to learn and share information, resources and certification processes;
- Ability to adjust work priorities per program needs;
- Availability to work weekends and evenings as needed;
- Host various in-person and web conferences to facilitate group discussion and skills demonstration;
- Perform needs assessments with mentees and co-develop action plans to ensure the services provided are on track;
- Attend all team meetings, trainings and events;
- Schedule mentor sessions, follow up sessions, track and document the provision of services in a timely manner;
- Use multiple computer systems to track and review documentation in our database and perform daily tasks;
- Properly document and track data in various electronic databases;
- Discuss mentees’ concerns and trends with the Leads and Managers;
- Serve as an energetic and enthusiastic mentor for the Home Care Aides;
- Additional duties as assigned by your manager;
- Proactively make outreach phone calls to offer support and guidance to other Caregivers on various needs;
- Recruit, coach and support other caregivers over the phone.
