Our company is looking for a Advanced Practice Nurse to join our team.

Responsibilities:

  • Provide clinical coaching, consultation on clinical/case management issues for APNs and RNs at meetings and on an as-needed basis;
  • Assist in assigning duties based on education, training competencies;
  • Participation in the planning and budgeting for the clinical/nursing service;
  • Provide clinical care of patients;
  • Provides patient education to meet the needs of the patient/family;
  • Participates in clinical training along with other CARE Center and COE staff;
  • Able to provide appropriate treatment for patients on comfort care/hospice;
  • Collects relevant data and interacts with collateral contacts in the community and hospital, as necessary;
  • Has end of life/goals of care discussions with patients and families;
  • Prescribes or recommends medication or other forms of treatment, as clinically indicated;
  • Helps to de-escalate crisis situations and manages interpersonally difficult situations;
  • Documents all aspects of patient care in EMR system;
  • Informs and instructs patients/significant others on patient’s health/illness state and plan of care;
  • Coordinates appropriate referrals to specialties for patient care consultation, including primary care and infectious disease clinics;
  • Serves as liaison with community providers and hospital colleagues. Attends off-site meetings with the COE team, when appropriate.

Requirements:

  • Must have two years of administrative experience;
  • Licensed Advanced Practice Nurse or Certified Nurse Midwife in New Jersey;
  • Experience with electronic health records is a must;
  • Licensure must be in the state of practice, in good standing with no active sanctions;
  • Must have a minimum of 3 years OB/GYN or related public health nursing experience.
