Certified Home Health Aide (CHHA) Job Description Template
Our company is looking for a Certified Home Health Aide (CHHA) to join our team.
Responsibilities:
- Complete 12 hours of in-service training per year;
- Grocery shopping/errands;
- Act quickly and responsibly in cases of emergency;
- Follow the care plan of our RN supervisor;
- Prepare meals following patient nutritional requirements and preferences;
- Provide companionship;
- Accompany to appointments;
- Monitor for safety;
- Performs activity in a manner consistent with State regulations and with home health aide training;
- Performs activities in a manner consistent with home health aide training and as outlined by the RN case manager;
- Assists patients with personal care, transfers in and out of bed, ambulation, and with use of the bedpan;
- Maintains a positive and professional image;
- Assists with medications that are ordinarily self-administered;
- Assists with prescribed exercises which patients and home health aides have been taught by appropriate health personnel;
- Performs light housekeeping duties directly associated with patient care.
Requirements:
- Professional appearance and positive attitude;
- Must have legal work authorization for the United States;
- Ability to perform all essential job functions;
- Take pride in providing high quality care;
- Must be in good health and satisfy our medical requirements;
- Must have reliable transportation to get to assignments;
- Must be NJ Certified Home Health Aide;
- Enjoy working in a one-on-one setting, eager to improve and continuously learn new skills.
