Certified Nursing Assistant (CNA) / Home Health Aide (HHA) Job Description Template
Our company is looking for a Certified Nursing Assistant (CNA) / Home Health Aide (HHA) to join our team.
Responsibilities:
- Report concerns about Residents to Supervisor and coworkers;
- Provide transportation services, using company vehicle when requested;
- Document information pertaining to changes in Residents’ functional status, personal needs, appointments, etc;
- Practice safety in working and with use of equipment, chemicals, tools and utensils;
- Carry out other duties as assigned by Supervisor;
- Respond to Resident emergencies; provide first aid assistance and arrange for appropriate medical attention;
- Follow the service plan in providing services to Residents;
- Encourage socialization among Residents;
- Monitor health, safety and well-being of Residents;
- Identify supplies needed to perform tasks and report needs to Supervisor;
- Chaperone activities outside the facility when requested;
- Assist in the development and updating of each Resident’s service plan;
- Assist Residents with activities of daily living; including bathing, dressing, eating, toileting, hygiene and mobility;
- Skin care;
- Turning and positioning.
Requirements:
- High School Diploma;
- Appropriate documents required by the state in which you practice in;
- Current CNA License or 75 Hour HHA Certificate;
- 1+ years of CNA/HHA experience in a supervised setting within the past 3 years;
- American Heart Assocation CPR;
- Certified CNA for the state in which you practice or 1 year HHA experience;
- 1+ years of CNA experience in a supervised setting within the past 3 years;
- bilingual;
- American Heart Association CPR.
