Our company is looking for a Clinical Liaison to join our team.
Responsibilities:
- Facilitates the admission process to and from hospital and long-term care based referral sources;
- Performs other duties as assigned;
- Oversees GIP patient admission process based on identified symptom management needs for that level of care;
- Works closely with facilities and hospitals to identify, transition, and admit patients appropriately for general inpatient services;
- Have a professional demeanor with excellent communications skills;
- Be flexible, self-driven, accountable for results, autonomous, yet supportive of team efforts;
- Prior sales or business development experience, preferred;
- Be a licensed Nurse or Respiratory Therapist with acute care experience;
- Have a valid driver’s license, proof of valid auto insurance, and active clinical license are required;
- Attend Pediatric Conferences to educate community on PSC services and to market to the community;
- Travel between Philadelphia, Lancaster, Point Pleasant and surrounding areas in Pennsylvania;
- Communicate with physicians/leadership of facilities regarding status changes of the children;
- Meet with families to educate family on Pediatric Specialty Care services and provide family a tour of the facility;
- Receive and assess all referrals and disseminate information to the clinical team;
- Track Pediatric Specialty Care residents during their hospitalizations to provide continuity of care upon the patients’ return to the facility.
Requirements:

- Be a licensed Nurse or Respiratory Therapist with acute care experience;

- Registered Nurse or Respiratory Therapy license with acute care experience (critical care, preferred);
- Have a valid driver’s license, proof of auto insurance, and active clinical license;
- Positive and solution-focused attitute;
- Passion for getting patients into the best level of care to support healing and recovery;
- Be an excellent communicator – understanding the importance of keeping customers and teammates informed on progress;
- Ability to work independently with minimal supervision;
- Patient-centered with excellent customer service skills;
- Must live in the Springfield, IL area.
