Direct Support Professional Caregiver Job Description Template
Our company is looking for a Direct Support Professional Caregiver to join our team.
Responsibilities:
- Ensuring individual safety and well-being;
- Assisting with personal care tasks;
- Fostering positive relationships between individuals supported and their community;
- Maintain records and documentation as necessary and appropriate;
- Assisting consumers by lifting, transferring, and when necessary, to prevent injury, physically managing them;
- Assist or provide consumers with all aspects of personal care;
- Provide constant supervision of all consumers to assure that they are safe and do not wander from the home or yard;
- Observe, document and report unusual behavior and/or incidents;
- Assure active treatment and implementation of Person Centered Plans;
- Medication administration;
- Assisting with home skills, such as preparing meals, housekeeping, and laundry;
- Supporting positive behaviors;
- Assisting with personal care and hygiene, such as bathing, dressing, and grooming;
- Providing a positive and fun environment;
- Maintaining records and data entries.
Requirements:
- Must be 18 years of age;
- Three or more positive references;
- Ability to relate with sensitivity to others;
- Willingness to take on tasks and duties as assigned;
- Physical examination including pre-employment and annual Tb tests;
- Ability to get along with people and to work as a team;
- Attend staff meetings and scheduled in-services and training’s;
- Attend training’s and pass exams (tests) as required;
- Evidence of ability to handle crisis situations with calmness and objectivity;
- High School Diploma, GED, or the ability to pass a basic competency exam;
- Must be 21 years of age;
- Must be at least 18 years of age (Per OAR 411-323-0050 (7)(a));
- Must have reliable transportation;
- Must be able to pass a criminal background check and drug test;
- Impeccable attendance.
