Our company is looking for a Home Health Registered Nurse (RN) to join our team.

Responsibilities:

  • Administers medication and IV fluids skillfully, safely and as required;
  • Initiates preventive and rehabilitative nursing procedures as appropriate;
  • Supervising LPNs and CNAs;
  • Counseling the client and family members in meeting nursing and related needs;
  • Make referral to other services as needed;
  • Administers client treatments and procedures in accordance with physician orders and NLCS nursing policies and procedures;
  • Initiating and updating CNA care plans;
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient;
  • Participates in interdisciplinary patient care conferences and Agency Performance Improvement processes;
  • Regularly re-evaluates patient nursing needs;
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts towards self-sufficiency and optional comfort care;
  • Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided;
  • Responsible for participating in on-call rotation and emergency call according to agency policy;
  • Drive an average of 200 miles per week in the Kennesaw, GA area;
  • Drive an average of 200 miles per week in the King, NC area.

Requirements:

  • Impeccable written and verbal communication skills;
  • Very strong clinical skills;
  • Experience with MS Office Suite and most EMR programs;
  • You are empowered and accountable for making a difference and leading your areas of responsibility;
  • Drive the clinical excellence, quality and integrity performance for the organization;
  • A minimum of one year of clinical experience is preferred;
  • Possess a valid state driver’s license and automobile liability insurance;
  • Serve as Registered Nurse the clinical care voice for patient care;
  • Is punctual for client visits and reports to work as scheduled;
  • Attends case conferences as requested and staff meetings as requested;
  • Participates in on-call coverage;
  • Active Applicable State Licensure;
  • Interacts with clients and families to meet individual physical, psychosocial, and spiritual needs;
  • Documents only for visits completed and uses highest ethical standards in documentation;
  • Complies with the requirements of state licensure and accreditation by following policy and procedure maintaining competencies and knowledge base.
