Home Health Registered Nurse Job Description Template
Our company is looking for a Home Health Registered Nurse to join our team.
Responsibilities:
- Case management and coordination;
- Other duties as assigned;
- Collaborate with physicians to create a designated care plan for each patient;
- Perform client assessments as necessary;
- Make home visits to clients in designated geographic territories;
- Participates in interdisciplinary patient care conferences and Agency Performance Improvement processes;
- Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided;
- Responsible for participating in on-call rotation and emergency call according to agency policy;
- Providing intermittent nursing services;
- Educating patients on disease processes and medication benefits and side effects;
- Travel within Westmoreland County;
- Initiating plans of care, re-evaluating patient’s needs and revising recommendations;
- Administer medications, provide vaccinations and render treatment to patients;
- Providing those services and/or treatments requiring substantial and specialized nursing skill;
- Have a good understand of Medicare guidelines.
Requirements:
- CPR Certification;
- CPR Certified;
- Possess a valid state driver’s license and automobile liability insurance;
- And the ability to work efficiently in rapidly changing environments;
- One year med/surg experience required;
- Valid Drivers’s License w/ Clean DMV;
- Have knowledge and understanding of the Medicare Conditions of Participation;
- Exemplary assessment skills and the ability to problem-solve independently within his/her scope of practice;
- No less than 1 year of home health and/or 2 years of med-surg experience;
- Possess a valid drivers license and auto insurance with reliable transportation;
- State clearances;
- Must have 1 year of recent Home Health experience;
- Must have current Home Health experience of at least 2 years – Required;
- Must have a valid GA License;
- California.
