Our company is looking for a Hospice Licensed Practical Nurse (LPN) to join our team.

Responsibilities:

  • Schedule: 12 hour shifts;
  • Paid Weekly;
  • Medical;
  • Palliative/Crisis care, supervision of CNA, STNA;
  • Contributes to the effectiveness of case management and coordination of services through open communication;
  • Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers;
  • Participates in staff and IDT meetings as requested;
  • Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company;
  • Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides;
  • Attends nursing home case conferences as requested;
  • Assists the RN and/or physician in performing specialized duties related to end-of-life care;
  • Follows directives of case manager and supports the efforts of case coordination;
  • Assists the patient in learning appropriate self-care techniques;
  • Delivers care to assigned clients and performs direct care in accordance with an established plan of care;
  • Administers medications and observes patients for adverse reactions to medications or treatments.

Requirements:

  • LPN License;
  • Clean Background Check and Clean Drug Screen;
  • Must have access to reliable transportation;
  • Current CPR, TB, Physical;
  • Assists the patient in learning appropriate self-care techniques;
  • Ensures the maintenance and confidentiality of client records and employee information in accordance with Confidentiality policy;
  • Follows directives of case manager and supports the efforts of case coordination;
  • Assists the RN and/or physician in performing specialized duties related to end-of-life care;
  • Attends nursing home case conferences as requested;
  • Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides;
  • Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company;
  • Contributes to the effectiveness of case management and coordination of services through open communication;
  • Participates in staff and IDT meetings as requested;
  • Prepares equipment and materials for treatment adhering to aseptic technique as required;
  • Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers.
