The Registered Nurse (RN), Hospice records patient histories, performs various diagnostic tests, administers medications, and develops patient care plans in conjunction with other medical professionals. Provides professional nursing care to assigned terminally ill patients in a hospice environment. Being a Registered Nurse (RN), Hospice observes patients’ condition and records observations in patient medical records. Performs examinations and prescribed procedures, operating and monitoring medical equipment as necessary. In addition, Registered Nurse (RN), Hospice requires graduation from an approved nursing program. Typically reports to a manager or head of a unit/department. Requires a state license to practice. Registered Nurse (RN), Hospice’s years of experience requirement may be unspecified. Certification and/or licensing in the position’s specialty is the main requirement.

Our company is looking for a Hospice Registered Nurse (RN) to join our team.

Responsibilities:

  • Regularly re-evaluates patient nursing needs;
  • Counsels the patient and family in meeting nursing and related needs;
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient;
  • Provides health care instructions to the patient as appropriate per assessment and plan of care;
  • Performs initial and ongoing nursing assessments;
  • Provides supervision of care provided by CNAs and LVNs and homemakers;
  • Performs specialized nursing skills related to palliative and end-of-life care related to managing discomfort and providing symptom relief;
  • Triages patient care problems;
  • Develops and maintains patient’s individualized plan of care;
  • Provides psychosocial support to patients and families;
  • Coordinates personal care, DME, and obtaining medications and medical supplies;
  • Provides preparation and support for patient’s death; pronounces deaths in accordance with state regulation, provides post mortem care;
  • Notes and carries out physician and nursing orders;
  • Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate;
  • Assesses and coordinates patient’s discharge planning needs with members of the healthcare team.

Requirements:

  • Assures the continuity of quality patient care delivered;
  • Reports to work and patient homes promptly as directed;
  • Abides by company policy and procedures as outlined by company policy manual;
  • Identifies deviations from normal Nursing and normal patient condition status. Initiates appropriate actions based on the deviation;
  • Documents medication regimen and updates medication profile according to established procedures;
  • Demonstrates commitment and professional growth;
  • Maintains current knowledge of, and promotes compliance with all federal, state and local regulations;
  • Demonstrates competency in all levels of home health nursing practice;
  • Participates or communicates in patient care conferences offering clinical suggestions and updates of patient’s progress towards goals;
  • Makes revisions to the plan of care in response to identified patient care issues and notifies the Clinical Supervisor;
  • Minimum one year experience in medical, oncology or hospice clinical nursing preferred;
  • Excellent assessment and teaching skills;
  • Able to cover large geographic area;
  • Reliable car with current insurance policy;
  • Able to make home visits year round.
