Hospice Registered Nurse Job Description Template
Our company is looking for a Hospice Registered Nurse to join our team.
Responsibilities:
- Develop and update plan of care with the IDT and others involved in the patients’ care;
- Promote a customer service oriented approach to care and customer relationships;
- Provide skilled nursing as determined by the plan of care;
- Participate in customer conflict resolution;
- Develop and achieve professional growth goals and objectives; review with supervisor on a regular basis;
- Participate in the orientation of new team members as assigned by the Clinical Director and/or Team Leader;
- Represent professional nursing and Grace Healthcare Services in a positive manner;
- Participation in team meetings;
- Monitor the recertification/reauthorization process;
- Participate in proactive patient calls and visits;
- Attend regularly scheduled in-service and educational conferences;
- Educate patient/family and other primary caregivers in appropriate care techniques;

Requirements:
- Communication Skills;
- Current clinical practice and a strong customer service focus;
- CPR Certified;
- NJ Registered Nurse License required;
- Possess a valid state driver’s license and automobile liability insurance;
- And the ability to work efficiently in rapidly changing environments;
- Current license as a registered nurse in a practicing state;
- Previous RN experience, preferred;
- A minimum of two (2) years acute care clinical experience is required;
- Home Health Experience preferred;
- Previous RN experience required;
- Must be able to position and/or transfer an average adult patient and possess gross coordination sufficient to perform patient care tasks as required;
- Must have availability of personal vehicle with proof of insurance;
- One year of relevant experience preferred;
- Licensure/certification/registration: Appropriate state RN licensure required. BLS Healthcare Provider required. ACLS, PALS preferred.
