Our company is looking for a Hospice Registered Nurse to join our team.

Responsibilities:

  • Develop and update plan of care with the IDT and others involved in the patients’ care;
  • Promote a customer service oriented approach to care and customer relationships;
  • Provide skilled nursing as determined by the plan of care;
  • Participate in customer conflict resolution;
  • Develop and achieve professional growth goals and objectives; review with supervisor on a regular basis;
  • Participate in the orientation of new team members as assigned by the Clinical Director and/or Team Leader;
  • Represent professional nursing and Grace Healthcare Services in a positive manner;
  • Participation in team meetings;
  • Monitor the recertification/reauthorization process;
  • Participate in proactive patient calls and visits;
  • Attend regularly scheduled in-service and educational conferences;
  • Educate patient/family and other primary caregivers in appropriate care techniques;

Requirements:

  • Communication Skills;
  • Current clinical practice and a strong customer service focus;
  • CPR Certified;
  • NJ Registered Nurse License required;
  • Possess a valid state driver’s license and automobile liability insurance;
  • And the ability to work efficiently in rapidly changing environments;
  • Current license as a registered nurse in a practicing state;
  • Previous RN experience, preferred;
  • A minimum of two (2) years acute care clinical experience is required;
  • Home Health Experience preferred;
  • Previous RN experience required;
  • Must be able to position and/or transfer an average adult patient and possess gross coordination sufficient to perform patient care tasks as required;
  • Must have availability of personal vehicle with proof of insurance;
  • One year of relevant experience preferred;
  • Licensure/certification/registration: Appropriate state RN licensure required. BLS Healthcare Provider required. ACLS, PALS preferred.
