Hospice RN Job Description Template
Our company is looking for a Hospice RN to join our team.
Responsibilities:
- Performs all aspects of patient care in an environment that optimizes patient patient safety and reduces the likelihood of medical/health errors;
- Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary;
- Performs admission assessment within 24 hours of referral;
- Develops a written plan of care for each patient in the assigned geographic caseload;
- Provides skilled nursing care based on Hospice Philosophy;
- Evaluates effectiveness of Plans;
- Advocates for clients in obtaining needed services;
- Implements Plans of Care;
- Effectively analyzes situations, develops goals and Plans of Care;
- Performs a complete health assessment;
- Reassesses health care needs on an ongoing basis;
- Assesses and coordinates patient’s discharge planning needs with members of the healthcare team;
- Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate;
- Notes and carries out physician and nursing orders;
- Serves as the primary coordinator of all disciplines for well coordinated patient care.
Requirements:
- Registered Nurse with current licensure to practice professional nursing in Wisconsin;
- Preferred experience in Public Health, Home Care or Hospice nursing;
- Self-directed and able to work with minimal supervision;
- Graduate of an accredited School of Nursing;
- Minimum one year clinical experience;
- Ability to organize and prioritize workload independently;
- Demonstrates excellent observation, problem-solving, verbal and written communication skills;
- Past experience in hospice homecare of at least 1 year;
- Advocates for client in obtaining needed services;
- Evaluates effectiveness of Plans;
- Knowledge of hospice specialized care demonstrated by work history in one of the following: At least 2 years of oncology nursing;
- Implements Plans of Care;
- Experience utilizing a laptop and/or personal computer, preferred;
- Care of terminally ill clients in an institution (i.e. hospital, ECF) within the past year;
- Minimum Requirements: Registered Nurse, currently holding an unencumbered license to practice in the state; BSN preferred.
