Responsibilities:
- Coordinates discharge planning needs with members as needed;
- Instructs and educates patients and families;
- Orients and mentors new staff members;
- Monitors, records and communicates patient condition as appropriate;
- Follows Standard Precautions using personal protective equipment;
- Administers medications, changes dressings, and cleans wounds;
- Serves as the primary coordinator of all disciplines for patient care;
- Carries out physician orders and assesses plans to administer patient care;
- Utilizing computerized documentation systems and monitors vital signs;
- Provides age and culturally appropriate care.
Requirements:
- Provides for patient safety at all time;
- Can manage multiple priorities;
- Utilizes all equipment in the unit appropriate to patient with maximum efficiency and effectiveness based on knowledge and education;
- Adapts to a rapidly changing work environment;
- Interventions are implemented in a safe and appropriate manner and documented;
- Provides health care teaching to patient/family or significant other with documentation;
- Verifies accuracy of medication administration by following all safety measures;
- Demonstrates the ability to work with people form diverse socio-economic backgrounds.
