LPN Care Coordinator Job Description Template
Our company is looking for a LPN Care Coordinator to join our team.
Responsibilities:
- Performs other duties as assigned;
- Serves as a liaison for management staff, administration, physicians, managed care companies, and other customers;
- Works with the practice leadership to implement evidence-based practice guidelines as outlined by clinic policies and providers;
- Provides emergent staffing needs, e.g. room patients, schedule appointments, document patient encounters in EHR, etc. as directed.
Requirements:
- Strong written and verbal communication;
- Flexible schedule, including nights and weekends;
- Excellent organizational skills;
- Prefer prior experience in assisted living setting, but willing to train the best candidate;
- Must possess a current and active license to practice as an LPN in Michigan.
