Our company is looking for a LPN Care Coordinator to join our team.

Responsibilities:

  • Performs other duties as assigned;
  • Serves as a liaison for management staff, administration, physicians, managed care companies, and other customers;
  • Works with the practice leadership to implement evidence-based practice guidelines as outlined by clinic policies and providers;
  • Provides emergent staffing needs, e.g. room patients, schedule appointments, document patient encounters in EHR, etc. as directed.

Requirements:

  • Strong written and verbal communication;
  • Flexible schedule, including nights and weekends;
  • Excellent organizational skills;
  • Prefer prior experience in assisted living setting, but willing to train the best candidate;
  • Must possess a current and active license to practice as an LPN in Michigan.
