Our company is looking for a MDS Coordinator to join our team.

Responsibilities:

  • Audit resident charts, care plans, and charting to ensure documentation supports the care provided and reimbursement level;
  • Complete and transmit MDS assessment and reports in accordance with Medicare, OBRA , Medicaid and other benefit program requirements Benefits;
  • Ensure compliance with guidelines for participation in Medicare, Medicaid and other benefit programs;
  • Maintains a viable working relationship with all disciplines involved in the planning of resident care;
  • Identify and communicate need for Significant Change in status assessments;
  • Complete Discharge Assessment and entry/re-entry tracking forms per Federal regulations;
  • Schedules MDS assessments per guidelines, as assigned and maintains a monthly calendar;
  • Notify members of the IDT of changes or additions to monthly Care Plan Meeting schedule, for new admission, change of condition or interim meetings;
  • Electronically submit MDS information. Review, revise and correct, and resubmit as appropriate;
  • Contribute to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care;
  • Complete MDS/CAAs timely per schedule;
  • Informs members of the Interdisciplinary Team with the ARD and MDS’s due;
  • Assist with necessary completion of the nursing portion of the PPS MDS/CAAs in the absence of the Medicare MDS Coordinator as needed;
  • Audit MDS assessments and CAAs from all disciplines and notify discipline of any discrepancies;
  • Participates in facility QA initiatives.

Requirements:

  • Clinical experience in Long Term Care and/or Medicare setting;
  • Current license as a Registered Nurse in the state of Louisiana;
  • PDPM knowledge highly preferred;
  • CPR;
  • Excellent analytical and deductive reasoning skills;
  • RAC-CT Preferred;
  • Comply with the Residents’ Rights and Facility Policies and Procedures;
  • Excellent technical, assessment, documentation, and writing skills;
  • Perform Essential Duties as outlined above;
  • Good communication skills with excellent self-discipline and patience;
  • Minimum three (3) years of clinical experience in a health care setting. Minimum of two (2) years experience in a long-term care setting;
  • Education: Current RN license;
  • Registered Nurse or Licensed Practical/Vocational Nurse with required state licensure;
  • Excellent written and verbal communication skills and critical thinking abilities,
  • Current CPR certification and additional certification in nursing specialty desired.
