Our company is looking for a Nurse Practitioner II to join our team.

Responsibilities:

  • Integrates research to promote evidence-based practice for patients with complex acute, critical, and chronic illness;
  • Uses communication techniques that reflect attention to the needs of the patient/family based on patient age and other special needs;
  • Participates in medical and advanced practice research projects;
  • Develops, implements and evaluates the plan of care based upon individual patient and family assessment including age and other specific needs;
  • Acts as consultant for triage nurses;mperforms phone consultation with outpatients;
  • Integrates the nurse practitioner role into systems, processes, and decision making to function fully within the health care team;
  • Performs independent follow-up of patients in the clinic setting;
  • Prepares pre-op orders as appropriate on patients prior to surgery or endovascular procedures;
  • Maintains current knowledge regarding state and federal regulations impacting NP practice;
  • Adapts performance of skills to meet the age and other population specific needs for patients/customers served;
  • Analysis of data reflects attention to age and other specific variables;
  • Formulates the appropriate differential diagnoses based on the history, physical exam, and clinical findings;
  • Synthesizes data from a variety of sources to make clinical judgments and decisions about appropriate recommendations and treatments;
  • Acts as a liaison between the CVSD and outreach sites, providing staff consultation and education as needed;
  • Promotes collaboration among members ofthe multidisciplinary health care team to facilitate optimal care for complex acute and chronic patients.

Requirements:

  • Sound knowledge of adult cardiovascular health, and related diagnostic and clinical therapies;
  • Ability to teach patients, families, staff, and students; knowledge of research methodologies;
  • Advanced physical and psychosocial health assessment skills;
  • Excellent teamworking abilities; professional image via appearance and conduct;
  • Competent to function as preceptor;
  • Ability to communicate effectively with patients, staff, physicians, and the public;
  • Able to demonstrate ability to deal with high acuity/complexity and rapidly changing situations;
  • Ability to work as part of a physician/nurse practitioner dyad.
