Our company is looking for a Palliative Care Nurse Practitioner to join our team.

Responsibilities:

  • Assists with the provision of palliative care in accordance with facility, state, and federal regulations;
  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs;
  • Implements interventions to support the patient with chronic conditions to decrease recurring hospitalizations;
  • Collaborates with primary care physicians/NP by making appropriate referrals, follow up calls, and coordination of care;
  • Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients;
  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities;
  • Monitors the effectiveness of interventions;
  • Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients.

Requirements:

  • Will consider less NP experience with RN hospice and/or homecare experience;
  • Minimum of 5 years OR clinical hospice and palliative care experience. 2 years’ experience in the Nurse Practitioner role preferred;
  • Must work well independently and within a team;
  • Strong computer skills, including Word, Excel, and PowerPoint;
  • At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN);
  • Current Advanced Practice Registered Nurse (APRN) Licensure;
  • Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful;
  • Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position;
  • Graduate of an accredited School of Nursing;
  • Knowledge of clinical standards of care;
  • Education and/or Experience: Completion of Nurse Practitioner accredited program;
  • Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems;
  • Minimum of two (2) years clinical experience, desired;
  • Exceptional interpersonal skills & superb communication a must;
  • At least one (1) year of experience in Hospice or Palliative Care.
