Physical Therapist PRN Job Description Template
Our company is looking for a Physical Therapist PRN to join our team.
Responsibilities:
- Maintains documentation of services in the medical record;
- Appropriately assign and supervisespecific treatment procedures to physical therapy assistants or aides;
- Evaluates, records and reports on patient’s response to treatment and progress toward identified goals;
- Provides age and culturally appropriate care;
- Follows Standard Precautions using personal protective equipment as required;
- Ability to complete the following activities safely, and at times, for prolonged periods;
- Instructs and educates patients and families;
- Current license in the state of practice;
- Demonstrated competency in applying the principles, methods, materials and equipment used in physical therapy;
- Certification, if applicable, and demonstrated competency in identified areas of specialization;
- Excellent interpersonal, oral and written communication skills;
- Coordination of care with entire care team;
- Preparing timely, complete and accurate clinical notes;
- Establishe, revise and evaluate a plan of care which is appropriate to problems identified and involves the patient/family;
- Perform initial and on-going assessments of patient’s condition.
Requirements:
- Graduate of an accredited college or university with a Bachelors degree in Physical Therapy. Also Referred to as: RPT, RPT/L, PT, MSPT, MPT, DPT;
- Interest in sub-acute, geriatric rehabilitation and general rehabilitation;
- Licensure as a Physical Therapist necessary to work in Long Term Care;
- Education and/or Experience: Two years (preferred) of clinical experience in the field of physical disabilities working as a Therapist;
- Bachelor’s degree in Physical Therapy from an accredited program;
- Must be able to push/pull 5-50 lbs;
- Must possess dexterity of arms, hands, and fingers bilaterally;
- Electronic Medical Record (EMR) experience preferred; Cerner strongly preferred;
- Current BLS;
- Graduate of an accredited school of Physical Therapy;
- Possessing a Physical Therapy degree and licensed in the state of Florida;
