Our company is looking for a Physical Therapist to join our team.

Responsibilities:

  • Meet with patients and identify needs and goals;
  • Maintain patient confidentiality;

  • Maintain professional and technical knowledge through attending educational workshops; reviewing professional publications, etc;
  • evaluations including short and long term goals, collaboration demands of other staff members, and required patient and caregiver involvement;
  • Implements IFSP for enrolled families and recommends frequency of services based on child development status and family priorities and concerns;
  • Consult with physicians regarding change in treatment;
  • Works to return the individual to optimum and productive level within the client’s capabilities;
  • Diagnoses movement dysfunction and develops a treatment plan;
  • Prepares and submits clinical and progress summaries based on the attainment of goals;
  • Writing progress, evaluation, and discharge reports;
  • Assists physician with evaluating level of function;
  • Collaborate with director of nursing and/or social worker on Medicare admissions;
  • Coordinating with interdisciplinary care plan team to develop long term treatment and discharge plan;
  • Consults with patients to learn about their physical condition and symptoms.

Requirements:

  • Must own a car, provide proof of car insurance and willing to travel throughout San Francisco Bay Area to work with patients;
  • Some experience is preferred, but new grads are welcome to apply;
  • Active Physical Therapist License in the State;
  • Minimum of one year (1) experience as a Physical Therapy Assistant preferred;
  • A thorough knowledge of current treatment practices;
  • Knowledge of Occupational Therapy and an understanding of when these techniques can benefit patients;
  • Experience with recommending assistive equipment to patients and outpatient or home health follow-up programs;
  • Able to bend, kneel, squat, stand, and lift heavy objects as needed;
  • Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions;
  • Ability to utilize electronic medical record and Microsoft Office programs;
  • Exceptional time management skills in performing all assigned tasks;
  • Pediatric experience with the birth to three populations is preferred;
  • Current BLS Certification;
  • Must have a Master of Science or Doctoral Degree in Physical Therapy with professional licensure in the state in which he/she is employed;
  • CPR Certification.
