Our company is looking for a Registered Nurse / RN – Home Health | Full Time to join our team.
Responsibilities:
- Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided;
- Responsible for participating in on-call rotation and emergency call according to agency policy;
- Participates in interdisciplinary patient care conferences and Agency Performance Improvement processes.
Requirements:
- Documents only for visits completed and uses highest ethical standards in documentation;
- Complies with the requirements of state licensure and accreditation by following policy and procedure maintaining competencies and knowledge base;
- Interacts with clients and families to meet individual physical, psychosocial, and spiritual needs;
- Participates in on-call coverage;
- Attends case conferences as requested and staff meetings as requested;
- Serve as Registered Nurse the clinical care voice for patient care;
- You are empowered and accountable for making a difference and leading your areas of responsibility;
- Possess a valid state driver’s license and automobile liability insurance;
- A minimum of one year of clinical experience is preferred;
- Complete all mandatory agency education and meetings in the time required unless arrangements have been made with an immediate supervisor;
- Other duties as assigned;
- Accept assignments for patient admission to home health and subsequent visits as required;
- Assume responsibility to remain current in the nursing skills and theory as it pertains to the delivery of home health services;
- Working knowledge of Medicare/Medicaid regulations, concepts in managed care and Joint Commission requirements;
- Provide instruction to both patients and significant others identified as care participants.
