Our company is looking for a RN Care Coordinator to join our team.

Responsibilities:

  • Act as the clinical support for the community health workers;
  • Develop targeted care plans;
  • Oversee systems for identifying high risk patients through EHR, provider referrals, registries, and health insurance payers;
  • Provide home visits;
  • Manage patient registries;
  • Updates and documents in Electronic Patient Record and Electronic Patient Management system, in an accurate, timely and efficient manner;
  • Directly supports SCPA’s highest risk patients in collaboration with other members of the healthcare team;
  • Short-term and long-term disability, basic life, and accidental death and dismemberment insurance;

Requirements:

  • Pediatric clinical experience preferred;
  • Working knowledge of Medicaid, commercial and third party payers preferred;
  • CA Registered Nurse (RN) License required;
  • Demonstrated ability to apply analytical and problem solving skills;
  • Strong interpersonal and written communication skills are essential;
  • Minimum of 3 years nursing experience in an acute hospital setting;
  • Ability to work efficiently in a fast-paced environment with changing priorities;
  • Masters Degree in nursing with a concentration in Case Management can serve as a substitute for the experience requirement;
  • Current PA Registered Nurses’ License;
  • Current licensure in Massachusetts as a Registered Nurse required;
  • Evidence of continuing professional development;
  • Excellelent computer skills including managing work against performance metrics and reporting on key indicators important to the department;
  • Current PA Registered Nurses’ License;
  • Demonstrated skills in organizing and directing staff teams to complete assignments accurately and on time;
  • Recent experience in acute care setting involved with clinical activities and/or a managed care environment working in case management.
