RN Care Coordinator Job Description Template
Our company is looking for a RN Care Coordinator to join our team.
Responsibilities:
- Act as the clinical support for the community health workers;
- Develop targeted care plans;
- Oversee systems for identifying high risk patients through EHR, provider referrals, registries, and health insurance payers;
- Provide home visits;
- Manage patient registries;
- Updates and documents in Electronic Patient Record and Electronic Patient Management system, in an accurate, timely and efficient manner;
- Directly supports SCPA’s highest risk patients in collaboration with other members of the healthcare team;
- Short-term and long-term disability, basic life, and accidental death and dismemberment insurance;

Requirements:
- Pediatric clinical experience preferred;
- Working knowledge of Medicaid, commercial and third party payers preferred;
- CA Registered Nurse (RN) License required;
- Demonstrated ability to apply analytical and problem solving skills;
- Strong interpersonal and written communication skills are essential;
- Minimum of 3 years nursing experience in an acute hospital setting;
- Ability to work efficiently in a fast-paced environment with changing priorities;
- Masters Degree in nursing with a concentration in Case Management can serve as a substitute for the experience requirement;
- Current PA Registered Nurses’ License;
- Current licensure in Massachusetts as a Registered Nurse required;
- Evidence of continuing professional development;
- Excellelent computer skills including managing work against performance metrics and reporting on key indicators important to the department;
- Current PA Registered Nurses’ License;
- Demonstrated skills in organizing and directing staff teams to complete assignments accurately and on time;
- Recent experience in acute care setting involved with clinical activities and/or a managed care environment working in case management.
