Our company is looking for a Rn/Lpn to join our team.

Responsibilities:

  • Resolves patient problems and needs by utilizing multidisciplinary team strategies;
  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families;
  • Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs;
  • Contributes to team effort by accomplishing related results as needed;
  • Discharge planning for Home Care patients;
  • Attends mandatory meetings;
  • Observes/ensures compliance with Medicare and State guidelines;
  • Admit, transfer and discharge residents according to policy;
  • Other duties as assigned;
  • Follow established safety and infection control procedures;
  • Direct and supervise the day-to-day functions of nursing assistants and other staff being supervised;
  • Communicate all pertinent resident information to unit staff at beginning of tour of duty and to oncoming nursing staff at end of tour of duty;
  • Perform emergency procedures such as CPR/Heimlich Maneuver, and in the event of death, notify family, coroner (if applicable) and funeral home;
  • Participates in all required inservice training;
  • Maintains safe working conditions and follows dress code.

Requirements:

  • BLS certified;
  • Clinical experience;
  • Current state licensure as a Registered Nurse (RN), in good standing;
  • Min 1 Year Homecare Experience;
  • Current state licensure as a Licensed Practical Nurse (LPN), in good standing;
  • Reliable Transportation and Auto Insurance is Required;
  • Minimum of 6 months experience in a long term care setting preferred;
  • Knowledge of supplies and equipment used by department and the care of this equipment;
  • Meet all health requirements imposed by law;
  • Ability to tactfully deal with facility personnel, residents, family members, visitors, etc;
  • Graduate of an accredited School of Nursing;
  • Knowledge of safety regulations;
  • Knowledge of current standards of nursing practice;
  • License in good standing with licensure board;
  • Ability to speak, write, read, and understand English.
