Our company is looking for a Rn Mds Coordinator to join our team.

Responsibilities:

  • Maintains current knowledge of RAI Manual and MDS assessment process;
  • Monitors and ensures completion of 7-day look-back documentation by nursing staff within prescribed MDS timeframe;
  • Initiates care plans and supporting activities that will result in best possible outcome for assigned residents;
  • Performs all job duties in a manner that ensures that confidential information and resident rights are protected at all times;
  • Promotes and participates in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current and new regulations;
  • Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents;
  • Conducts Utilization Review meeting. Ensures that all disciplines have updated and accurate information;
  • Participates in morning clinical meeting;
  • Completes assessments, Minimum Data Set (MOS} and care plans for all residents assigned;
  • Submit the MDS to the appropriate agency;
  • Maintain a schedule of due dates for MDS and Care Plans including new admissions and those with significant changes;
  • Be a resource for the LTC MDS Coodinator;
  • Maintain the confidentiality of all resident care information;
  • Ensure that the facility’s policies and procedures governing advance directives are reviewed with the resident and/or representative;
  • Report and investigate all allegations of resident abuse and/or misappropriation of resident property.

Requirements:

  • Strong clinical skills;
  • Valid CPR certification required;
  • Basic Computer skills;
  • Knowledge of restorative programs and implementation;
  • Assures that care plans are keep current and reflective;
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult people;
  • Strong organizational skills;
  • Excellent leadership skills;
  • Excellent decision making capabilities;
  • Must be able to read, write, speak, and understand the English language;
  • Monitoring of QM with emphasis on those impacting five-star rating;
  • Oversees care plan meeting with resident and family;
  • Must not pose a direct threat to the health or safety of other individuals in the workplace;
  • Proficiency in MDS 3.0 and a knowledge of PDPM;
  • Monitors documentation of Medicare and managed care resident and educate as needed.
