Transitional Care Coordinator Job Description Template
Our company is looking for a Transitional Care Coordinator to join our team.
Responsibilities:
- Current State License as a Registered or Practical Nurse preferred (as applicable per state and/or contract requirement);
- Consistently models the company’s philosophy of customer service and compliance;
- Has a strong understanding of clinical processes while maintaining compliance with Maxim, ACHC, State and Federal Guidelines;
- Demonstrates industry knowledge;
- Facilitates transition of care between patient and Clinical Teams once the patient or family chooses Maxim Healthcare;
- Routinely provides superior customer service and follow up to facility based referral sources in the area;
- Demonstrated ability to provide a high level of customer service;
- In accordance with Maxim policies, presents in-services to hospitals, physicians, and facility staff as requested;
- A minimum of two (2) years experience in nursing, home health preferred;
- Ability to handle a fast paced environment with a high level of organization;
- Performs other duties as assigned/necessary.
Requirements:
- Ability to view screen and enter data into a laptop computer (or similar hardware) within a standard period of time;
- This role is performed onsite at facilities or telephonically as directed by the manager;
- Ability to manipulate laptop computer (or similar hardware) between office and site settings;
- Must attend 2 weeks of onsite training at Corporate Office in Brentwood, TN;
- Ability to establish a home office workspace;
- Ability to sit for an extended time period (1 – 2 hours);
- Ability to communicate with clients and team members including use of cellular phone or comparable communication device.
