Registered Nurse (RN) - Home Health

We are seeking an experienced and compassionate Registered Nurse (RN) to join our growing Home Health team serving patients throughout Albuquerque and Rio Rancho.

This position is ideal for an RN who has a strong background in home health, is comfortable working independently in the field, and has hands-on experience completing Start of Care (SOC) assessments and OASIS documentation.

Active New Mexico Registered Nurse (RN) license in good standing

Minimum 2 years of total nursing experience

Minimum 1 year of Home Health nursing experience

*
Minimum 1 year of Start of Care (SOC) and OASIS experience

Ability to manage patient care independently in a home setting

*
Reliable transportation and ability to travel throughout the Albuquerque/Rio Rancho service area

As a Home Health RN, you will help patients receive high-quality clinical care while remaining safely and comfortably in their homes. Complete comprehensive Start of Care (SOC) assessments

Perform skilled nursing assessments and visits

*
Develop and update individualized plans of care

*
Monitor changes in patient condition and communicate with physicians and the interdisciplinary care team

*
Coordinate care with therapists, physicians, caregivers, and other healthcare professionals

*
Help identify changes in condition and opportunities to improve patient outcomes and prevent unnecessary hospitalization
