Pharmacist - Hospital Pharmacy

Clinical Pharmacy Services
Under the direction of the Clinical Coordinator or Manager and the Pharmacy Director; optimizes patient medications therapy by providing comprehensive clinical pharmacy services. Works collaboratively with the health care team to accomplish goals.
Job Requirements
Minimum Education
B.S. Pharmacy or Pharm D; or Master of Pharmacy
Continuing education as required for licensure by TSBP
Minimum Work Experience
~ Previous hospital experience with clinical responsibilities

Required Licenses/Certifications
Active registration as a licensed pharmacist in the state of Texas
Sterile Compounding Certification
BLS; ACLS; or PALS; depending on practice area
Registration as a preceptor with TSBP Required Skills; Knowledge; and Abilities
Assess; document; and perform physician consultations; excluding total parenteral nutrition; but including all medication dosing consults
Perform home medication reconciliation
Provide education presentations to pharmacy staff and other staff upon request
Ability to effectively direct and/or supervise personnel as appropriate
Ability to handle confidential information
Ability to maintain professional appearance and decorum at all times
Ability to demonstrate excellent oral and written communication skills
