Provide direct patient care to residents in our skilled nursing facility, including administering medications, wound care, and conducting vital sign checks
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor and report changes in resident condition to the healthcare team and physicians
Maintain accurate and up-to-date records of patient care and treatment
Collaborate with therapy staff to ensure successful rehabilitation outcomes
Participate in quality improvement initiatives to continuously improve patient care and satisfaction
